Creekside Sleep Medicine Center is dedicated to a comprehensive approach to your sleep complaints.

 

This begins with a thorough evaluation of your sleep issues by one of our Board Certified physicians. They take the time to teach patients about their condition and encourage them to participate in decisions about their care. An overnight sleep test may be suggested.

 

Our facility uses state-of-the-art diagnostic equipment in a comfortable environment. We have a team of experienced professionals to help you through the process of testing and treatment.

Dr Teresa Jacobs

Sleep Care Physician

Board certified in Sleep Medicine, Dr Jacobs uses her experiences in primary care, pulmonary medicine, and the intensive care unit in making a thorough assessment of a patient's situation. Dr Jacobs carefully listens to her patients and believes that closely working together with them achieves the best results. She provides clear information about treatment choice to guide in identifying the best option for their individual circumstance.  Dr Jacobs works closely to coordinate care with other providers. She is interested in all aspects of sleep health, focusing on adolescent and adult patients.

Kim Lonergan, ARNP

Sleep Care Physician

Kimberlee joins Creekside Sleep Medicine Center with a background in sleep medicine covering more than 25 years of research and clinical work. She is an Advanced Registered Nurse Practitioner and graduated magna cum laude with a Masters Degree in Nursing. Kimberlee utilizes her broad base of knowledge and experience to work together with her patients to diagnose and treat sleep disturbances. She believes that teamwork, communication, collaboration with staff and colleagues are all essential in helping her patients to attain optimal health. 

Dr Nicole N. Phillips

Dr Nicole Phillips

Nicole Phillips M.D. is a Chicagoland native. She moved to Seattle to complete a Neurology Residency at the University of Washington. Following her passion for Sleep Medicine, she subsequently completed a Sleep Medicine fellowship at the University of Michigan in Ann Arbor. She is Board Certified in Sleep Medicine and Neurology. Dr. Phillips believes that healthy sleep is essential for optimizing daytime function and overall quality of life. She recognizes that every patient is unique, and that medical, psychological, biological, social, and environmental factors influence an individual’s sleep duration and quality. She enjoys collaborating with her patients in order to establish the most effective and individualized treatment plans. She enjoys all aspects of clinical sleep medicine, with emphasis on treating sleep apnea, and working with other subspecialty groups to investigate alternatives to CPAP therapy. She is also interested in optimizing sleep for patients with insomnia, and improving sleep quality in the elderly/aging population. Dr. Phillips devotes much of her free time to long distance running and marathon training. She is also passionate about working with animals, and volunteers as an Adoptions Counselor at a local animal shelter.
